Release of Liability and Waiver Form

1.      Acknowledgement of Risk

I, the undersigned, acknowledge that participation in activities organized by the Columbus Latin Dance Academy involves inherent risks, including but not limited to physical injury, property damage, or other personal harm. I understand that these risks cannot be entirely eliminated, and I voluntarily assume all risks associated with my participation.

2.       Waiver and Release

In consideration of being allowed to participate in activities organized by the Columbus Latin Dance Academy, I agree to release and hold harmless the Columbus Latin Dance Academy, its affiliates, officers, employees, instructors, volunteers, and agents from any and all claims, demand, actions, or causes of action arising from or related to my participation. This release includes, but is not limited to, claims for person injury, property damage, or any other loss.

3.       Medical Condition

I certify that I am in good health and have no medical conditions or physical limitations that would preclude me from participating in activities organized by the Columbus Latin Dance Academy. I have consulted with a physician if necessary and have obtained medical clearance to participate. 

4.      Photography and Media Release

I grant permission to the Columbus Latin Dance Academy to take photographs, video recordings, or other media of me during activities organized by the Academy. I understand that these materials may be used for promotional purposes, including but not limited to social media, websites, and marketing materials. I waive any right to compensation of royalties for the use of these materials. 

5.      Governing Law

This Release of Liability and Waiver Form shall be governed by and construed in accordance with the laws of the state of Ohio, without regard to its conflict of law principles.

6.      Severability

If any provision of the Release of Liability and Waiver Form is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

7.      Acknowledgment

I have read and understood this Release of Liability and Waiver Form. I am aware that by signing this form, I am waiving certain legal rights and agree to be bound by its terms, and consent to receive emails from the Columbus Latin Dance Academy about upcoming events, workshops, socials, classes, and special discounts.
