Release of Liability, Waiver, and Code of Conduct

1. Acknowledgment of Risk

I, the undersigned, acknowledge that participation in activities organized by Columbus Latin

Dance Academy involves inherent risks, including but not limited to physical injury, property

damage, or other personal harm. I understand that these risks cannot be entirely eliminated,

and I voluntarily assume all risks associated with my participation.

2. Waiver and Release

In consideration of being allowed to participate in activities organized by Columbus Latin

Dance Academy, I agree to release and hold harmless Columbus Latin Dance Academy, its

affiliates, officers, employees, instructors, volunteers, and agents from any and all claims,

demands, actions, or causes of action arising from or related to my participation. This release

includes, but is not limited to, claims for personal injury, property damage, or any other loss.

3. Medical Condition

I certify that I am in good health and have no medical conditions or physical limitations that

would preclude me from participating in activities organized by Columbus Latin Dance

Academy. I have consulted with a physician if necessary and have obtained medical clearance

to participate.

4. Photography and Media Release

I grant permission to Columbus Latin Dance Academy to take photographs, video recordings,

or other media of me during activities organized by the Academy. I understand that these

materials may be used for promotional purposes, including but not limited to social media,

websites, advertising, and marketing materials. I waive any right to compensation or royalties

for the use of these materials.

5. Governing Law

This release of liability and waiver form shall be governed by and construed in accordance with

the laws of the State of Ohio, without regard to its conflicts of law principles.

6. Severability

If any provision of this release of liability and waiver form is found to be invalid or

unenforceable, the remaining provisions shall continue in full force and effect.

7. Acknowledgment

I have read and understand this release of liability and waiver form. I am aware that by

acknowledging this waiver, I am waiving certain legal rights and agree to be bound by its

terms.

8. Zero Tolerance Policy

Columbus Latin Dance Academy maintains a zero-tolerance policy for any form of

discrimination, harassment, or misconduct. All students are expected to treat other students,

staff, and instructors with respect and professionalism at all times.

This includes, but is not

limited to:

● Discrimination based on race, ethnicity, gender, sexual orientation, religion, disability, or

any other protected status.

● Sexual harassment or misconduct, including unwanted physical contact, inappropriate

comments, or any unwelcome romantic or sexual advances without consent.

● Bullying, intimidation, or verbal abuse in any form.

● Retaliation against anyone who reports or voices concerns about inappropriate

behavior.

● All instructors and students are required to follow the Academy’s safety and sexual

misconduct protocol. Any violation of this policy will result in immediate termination of

participation in Columbus Latin Dance Academy events and may lead to further legal

consequences.
